Do you need a deep cleaning — or just a regular cleaning?

A routine dental cleaning and polishing works above and just at the gum line — the right choice when your gums are healthy and you're maintaining them every six months. A deep cleaning goes further: it treats the space under the gums, where tartar has collected inside pockets that a toothbrush — and a regular cleaning — cannot reach.

When we measure your gums, pockets deeper than 3 mm are the usual signal that scaling and root planing is needed. Research suggests nearly half of adults over 30 have some degree of periodontitis, and most don't realise it — gum disease rarely hurts in its early stages, which is exactly why it costs people teeth.

Deep cleaning is likely the right treatment for you if:

The Procedure

What happens during scaling and root planing at Arasu

Your treatment starts with periodontal charting: we measure the pocket depth around each tooth and take X-rays where needed, so we treat what is actually there rather than working blind. This assessment is reviewed by our periodontist, who holds an Associate Fellowship in Laser Dentistry from the World Clinical Laser Institute (USA) and handles our flap surgery and grafting cases. Deep cleaning is her department's everyday work.

Scaling

We numb the area with local anaesthesia, then remove plaque and hardened tartar from the tooth surface and from inside the gum pockets, right down to the base. We use ultrasonic scalers alongside fine hand instruments — studies show ultrasonic cleaning achieves comparable results to manual scaling while taking 20–50% less time in the chair.

Root planing

We then smooth the root surfaces. A smooth root gives bacteria less to cling to and lets the gum reattach snugly against the tooth — which is how pocket depth reduces and healing holds. This is the step that turns a cleaning into genuine periodontal therapy.

We usually treat the mouth in halves or quadrants across two visits — occasionally up to four for severe build-up — so you're never numbed everywhere at once and each area gets unhurried attention. The first 24–72 hours

Expect mild gum soreness and some sensitivity to hot and cold — it settles on its own, and we'll tell you exactly what to rinse with and what to avoid. Stick to soft, lukewarm food for the first day and skip anything spicy or very hot.

Teeth may feel “different”

One thing that surprises patients: teeth can feel slightly loose or different for a few days. That isn't damage — it's the tartar that was splinting your teeth gone, and your gums beginning to tighten back.

We review you at 4–6 weeks, re-measure the pockets, and confirm the gums are reattaching as they should.

What it controls

Scaling and root planing controls gum disease — it stops the destruction and gives your gums the conditions to heal and reattach. Most chronic gum disease is successfully controlled with this procedure plus proper maintenance: cleanings every 3–4 months rather than the routine 6-month schedule, or the bacteria simply repopulate.

What it can't do

It does not reverse gum disease. Gum that has receded and bone that has been lost do not grow back with a cleaning. If pockets are very deep, scaling alone may not reach the base — those cases need dental flap surgery. Where recession has exposed roots, gum grafting is the treatment that covers them — a deep cleaning won't.

Smoking and uncontrolled diabetes also slow gum healing significantly; we factor both into your plan and follow-up schedule. Individual results vary based on the severity of gum disease, your home care, and these health factors — and we'll tell you at the assessment which category your case falls into.

The procedure is done under local anaesthesia, so the treatment itself is comfortable — most patients feel vibration and water spray rather than pain. Gums are usually a little sore for a day or two afterwards, which settles with the simple aftercare we give you.

No. Scaling removes deposits sitting on the tooth, not tooth structure. If teeth feel slightly loose afterwards, that's the hardened tartar — which was artificially propping them — now gone, and the gums beginning to tighten back around the teeth.

A normal cleaning maintains healthy gums by working above the gum line. A deep cleaning treats gum disease by cleaning inside the gum pockets and smoothing the roots below the gum line, usually under anaesthesia and across more than one visit.

If your gums respond well, you shouldn't need the full procedure repeatedly — but you will need maintenance cleanings every 3–4 months to stop the bacteria re-establishing. Skipping maintenance is the most common reason gum disease comes back.

Gums can reattach to a cleaned, smooth root — that's the goal of the procedure. Gum that has visibly receded, though, doesn't regrow on its own; covering exposed roots is done with gum grafting, which our periodontist also performs.
