Periodontist-led · for pockets scaling can't reach

Dental Flap Surgery in Coimbatore

When a deep cleaning has been given a fair chance and your gum pockets stay 6 mm or deeper, the infection is sitting where instruments can't reach it blind. Dental flap surgery — clinically, periodontal flap surgery — gently lifts the gum so your periodontist can clean the root and bone under direct vision, then secures the gum back so the pockets are shallower and the bone loss that loosens teeth is stopped. Here it is a planned, staged step performed by our periodontist — never a default first move.

Where To Start

Do you need flap surgery — or is a deep cleaning still enough?

For most gum disease, a deep cleaning (scaling & root planing) is the first-line treatment and does the job. Flap surgery is the next step only when pockets stay deep after deep cleaning has had a fair chance — because at that depth, instruments can no longer clean the root and bone reliably while working blind under the gum.

When we re-measure your gums and pockets remain at 6 mm or deeper, the infection is sitting where a cleaning can't reach — and lifting the gum to clean it under direct vision becomes the treatment most likely to save the teeth. It is a planned, staged decision, never a default.

Flap surgery may be the right step for you if:

Your gum pockets are still 6 mm or deeper after a professional deep clean.

Your gums still bleed easily, look red or feel swollen despite good home care.

An X-ray shows loss of the supporting bone around one or more teeth.

One or more teeth feel loose, or your gums have pulled away from the teeth.

You have persistent bad breath or a bad taste from trapped infection.

If pockets responded well to deep cleaning, you likely don't need surgery — and we'll tell you so.

These are genuinely different treatments — we won't push you up the ladder without cause. If your gums are still responding to non-surgical care, that's exactly what we'll recommend, and surgery stays on the shelf. Advanced gum disease sits at the deep end of our gum disease treatment pathway.

The Procedure

What happens during flap surgery at Arasu

Your treatment starts with periodontal charting and X-rays, so we operate on what is actually there rather than working blind. This plan is made by our periodontist, who holds an Associate Fellowship in Laser Dentistry from the World Clinical Laser Institute (USA) and performs our flap surgery and grafting cases. It is a real surgical procedure, but a routine, well-rehearsed one for her department.

Step 1

Numb & prepare

The area is fully numbed with local anaesthesia so you stay comfortable throughout — you feel pressure, not pain. Only the quadrant being treated is worked on at a time.

Step 2

Lift the flap

The gum is gently lifted away from the tooth as a flap, exposing the root surfaces and the true depth of the pockets — the access a blind cleaning simply can't give.

Step 3

Clean & reshape

Infection and hardened tartar are removed under direct vision; damaged bone is smoothed or grafted where indicated, and laser-assisted decontamination is added where it helps healing.

Step 4

Suture & heal

The gum is secured snugly back against the tooth with fine sutures so pockets are shallower and easier to keep clean, and you're sent home with a simple recovery plan.

We treat one quadrant per session — typically 45–90 minutes — so no area is rushed and you're never operated on everywhere at once. Where it helps, we use our laser gum therapy approach for cleaner cuts, less bleeding and faster healing — a minimally invasive option few clinics in Coimbatore offer with fellowship-trained hands. Where advanced disease has destroyed supporting bone, we can add a bone graft during the same procedure to rebuild lost support.

Flap surgery is a well-established, first-line surgical treatment for advanced periodontitis worldwide — turned to only when non-surgical care has been given a fair chance and the pockets remain too deep to clean any other way. Most cases are then held stable with proper periodontal maintenance.

Afterwards

Recovery — what healing really looks like

Flap surgery is a same-day procedure and most people are back to routine within a few days. Here's an honest day-by-day.

First 24 hours

Mild soreness as the anaesthetic wears off. Stick to soft, cool foods and avoid the surgical site when you brush. We'll tell you exactly what to rinse with and what to skip — and over-the-counter pain relief is usually all that's needed.

Days 2–3

Any swelling peaks and then settles. Most patients are back to work and their normal routine by now, keeping the area clean as advised.

Week 1–2

Gums begin to firm up. You come in for a quick review and, where sutures were used, their removal — and we confirm healing is on track.

Weeks 2–6

Pockets tighten and the gums fully heal against the cleaned roots. We set you up on a periodontal maintenance schedule to hold the result and stop the disease returning.

Smoking and uncontrolled diabetes slow gum healing significantly — we factor both into your plan and follow-up. Individual recovery varies with the extent of disease treated and your home care.

Straight Answers

What flap surgery can and cannot do

What it does

Flap surgery reaches the deep infection a cleaning can't, reduces pocket depth so you can keep the area clean at home, and stops the ongoing bone loss that loosens and eventually loses teeth. For the right case it is often the difference between keeping a tooth and needing an extraction — and where bone has been lost, a graft during surgery can help rebuild support.

What it can't do

It isn't a cure for gum disease — periodontitis is managed for life with maintenance, or the bacteria repopulate. Gum and bone already lost don't fully regrow on their own. If a deep cleaning is still working for you, surgery isn't needed yet — start with scaling & root planing. And where recession has exposed roots, gum grafting is the treatment that covers them — flap surgery won't.

How much a tooth can be saved depends on how much support has already been lost — the earlier advanced gum disease is treated, the better the outlook. We assess this honestly and tell you at the consultation which category your case falls into. Individual results vary.

Two clinics told me my back teeth had to come out. Here they charted the pockets, did flap surgery on two quadrants, and saved them. Honest, and a written estimate before starting.

SSuresh M.

The pockets stayed deep even after deep cleaning, so the periodontist explained why surgery was the next step. Felt pressure, not pain, and I was back at work in two days.

LLakshmi N.

Had flap surgery with a bone graft for a loose tooth. They were clear about what it could and couldn't fix. The tooth is firm again and the review was included.

DDeepa S.

As a diabetic I was nervous about gum surgery. They planned the visits carefully around it and the healing went smoothly. No upselling — they said one quadrant could wait.

Results vary based on the severity of gum disease, how much bone support remains, home care and health factors. Patient results shown may not be typical.

FAQs

Flap surgery — your questions answered

The procedure is done under local anaesthesia, so you feel pressure rather than pain during surgery. Afterwards there is usually mild soreness and some swelling for a few days, which settles with the simple aftercare and over-the-counter pain relief we advise.

Most patients are back to normal routine within 2 to 3 days. Any swelling peaks and settles over the first few days, and the gums firm up over the following weeks. We review you at 1 to 2 weeks for suture removal and to check healing.

Deep cleaning (scaling and root planing) works blind below the gum. When pockets are 6 mm or deeper, instruments cannot reliably reach the base of them. Flap surgery lifts the gum so the root and bone can be cleaned under direct vision — which is why it's the next step only after deep cleaning has been given a fair chance.

Flap surgery is specifically designed to stop the bone loss that loosens teeth and to give the gum the conditions to reattach more firmly. The earlier a loosening tooth is treated, the better the outlook — but whether a specific tooth can be saved depends on how much support has already been lost, which we assess honestly before surgery.

The gum is repositioned and reattaches against a cleaned root, and pockets become shallower. Gum and bone already lost don't fully regrow on their own — where bone needs rebuilding we add grafting during surgery, and where recession has exposed roots, gum grafting is the treatment that covers them.

Many plans cover periodontal surgery. We give you a written, itemised estimate and help you check your cover before you commit, and EMI is available for larger treatment plans.

Deep pockets that scaling cannot clean tend to keep the infection active, so bone loss continues and teeth loosen further over time. Flap surgery is recommended when it's the treatment most likely to save the teeth — we'll always tell you plainly if watching-and-maintaining is a reasonable alternative in your case.
