Does your child actually need the tooth taken out?

Baby teeth aren't just placeholders. They help your child chew and speak, and each back tooth quietly holds space for the adult tooth growing beneath it — so we don't remove one we can save. Decay is by far the most common reason a child's tooth ends up needing attention: research on Indian preschool children puts early-childhood tooth decay at around 47% — close to one child in two — but a decayed tooth can very often be repaired rather than removed. We recommend taking a tooth out only when one of these is true:

Decay too deep to repair — the tooth is broken down past the point a filling, a baby root canal or a crown can rescue it.

Infection or an abscess that won't settle — a gum boil, swelling or spreading infection, where removing the tooth protects the permanent tooth developing below.

Injury beyond saving — a tooth cracked, split or knocked loose in a fall or knock that can't be repaired.

An over-retained baby tooth (“shark teeth”) — the adult tooth is already erupting behind or in front while the baby tooth stubbornly stays put, so we clear the way.

Orthodontic space planning — occasionally your child's orthodontist asks us to remove one or more teeth in a planned sequence to guide crowded adult teeth into line.

At the visit we take a quick look and, where needed, a small X-ray to see the root and the adult tooth underneath. If the tooth can be kept, we'll say so and treat it. If it can't, we'll explain plainly why removing it is the better path for your child.

What happens when we remove your child's tooth

Most children are far more worried by the idea of an extraction than by the visit itself. Our job is to make the real thing calm and quick, and to keep your child feeling safe the whole way through. Here's how a typical appointment goes.

STEP 1

We prepare your child first

We use a simple “tell–show–do” approach — explain each step in words a child understands, show them the harmless bits, then do it, with no surprises. A little numbing gel goes on the gum first so the area is already dull.

STEP 2

A simple removal is quick

For a loose or straightforward baby tooth, we numb the area with local anaesthetic and gently ease the tooth out — most take only a few minutes, and your child feels pressure and wiggling rather than pain. This is the great majority of children's extractions.

STEP 3

A more involved removal, if needed

If a tooth is badly broken down at the gum line, or an adult tooth has grown around a baby root, it takes a little longer and a more careful technique. For these — or a very anxious child, or one with special healthcare needs — we can arrange sedation.

Results vary. How simple an extraction is depends on the tooth, the amount of decay, and your child's age and cooperation. We'll give you a clear picture after the check-up, not a one-size-fits-all promise — and we always talk you through what's planned and why before we begin.

The half most clinics skip

After the tooth is out: protecting the space

Removing a tooth is only half the job. When a back baby tooth comes out early, the teeth on either side start to drift into the gap — and they do it sooner than most parents expect. In Indian schoolchildren the first baby molar is the tooth most often lost early, and studies show the space begins closing within the first few months after it's gone. Once that space is lost, the adult tooth underneath can come through crowded, rotated or blocked out.

That's why early tooth loss matters for the long run: children who lose baby teeth prematurely are found to need orthodontic treatment far more often than those who don't — by one pooled estimate, close to five times as often. So after a back-tooth extraction we'll usually recommend a small, custom space maintainer — a simple appliance that holds the gap open until the adult tooth is ready. It's often what prevents a longer, costlier round of braces later on.

Not every extraction needs one. A front baby tooth that was already close to falling out on its own, for instance, usually doesn't. We'll tell you honestly whether your child needs a space maintainer or not — we don't fit appliances a child doesn't need.

Caring for your child at home

  • Have your child gently bite on the gauze pad for about 30–45 minutes to help the area settle.
  • Stick to soft, cool foods and drinks for the rest of the day — and no straws, vigorous rinsing, spitting or poking the socket, so the healing clot isn't disturbed.
  • Keep your child from biting their lip or cheek while the numbness wears off (this can take a couple of hours).
  • A children's dose of the usual over-the-counter pain reliever handles any soreness; a cold pack on the cheek eases mild swelling.
Mild soreness for a day is normal. If you notice heavy bleeding that won't stop, a fever, or swelling that's getting worse rather than better, call us — we're open every day until 8:30 PM and will see your child promptly.
