Advanced gum disease — treated to save your teeth

Periodontitis Treatment in Coimbatore

At Arasu Dental Care, periodontitis treatment stops advanced gum disease from destroying the bone and tissue that hold your teeth in place — so you keep your own teeth instead of losing them one by one. It’s the care you need once gum disease has moved past bleeding gums into deeper damage: gums pulling away from the teeth, a bad taste that won’t clear, teeth that feel loose or have begun to drift or splay.

This is the stage many people in Tamil Nadu know as pyorrhea (பயரியா) — and it’s exactly the kind of case our periodontist manages routinely. The earlier we see you at our R.S. Puram or Saibaba Colony branch, the more of your natural bite we can save.

Read the signs

Do these sound like you? The signs your gums need more than a cleaning

Gum disease is easy to ignore because it rarely hurts until it’s advanced. If you recognise several of the signs below, your gums have likely progressed beyond the early, reversible stage and need targeted treatment rather than a routine scale-and-polish:

Signs of advanced gum disease worth acting on

  • Gums that bleed when you brush, floss, or eat — not just occasionally, but most days. If bleeding is your main worry, our bleeding gums page covers it too.
  • Receding gums — teeth that look longer than they used to, or feel sensitive at the roots.
  • Persistent bad breath or a lingering bad taste that mouthwash doesn’t fix.
  • Loose or drifting teeth — teeth that feel mobile, or new gaps opening up between teeth that used to sit tight.
  • Pus, swelling, or tenderness around the gumline, or gums that feel sore when you chew.

The line that matters: the earliest stage, gingivitis, is inflammation of the gums only — and it can usually be reversed completely with a professional cleaning and better home care. Periodontitis is what comes next, once the infection has reached the bone and started to dissolve the support around your teeth. That lost bone doesn’t grow back on its own — which is why this stage is treated, then managed for life, rather than simply cured. If you’re not sure which stage you’re at, that’s the first thing we establish at your consultation.

Where you stand

How advanced is your gum disease?

Where you sit on this scale decides how much treatment you’ll need — from a single deep clean to surgical care. We assess it by measuring the pockets between your gums and teeth and, where needed, taking X-rays to see the bone level. Here’s what each stage looks like:

StageWhat’s happeningWhat you’d noticeTypical treatment
Gingivitis (reversible)Gums inflamed; no bone loss yetRed, puffy gums that bleed easilyProfessional cleaning + home care
Early periodontitisFirst bone loss around teeth beginsBleeding, mild recession, slight loosenessDeep cleaning (scaling & root planing)
Moderate periodontitisDeeper pockets, more bone lostBad breath, sensitivity, teeth starting to moveDeep cleaning ± laser or flap surgery
Advanced periodontitisSevere bone loss; teeth poorly supportedLoose teeth, gaps, pus, painful chewingFlap / regenerative surgery; some teeth may need replacing
This is a guide, not a diagnosis. Two people with the same symptoms can be at very different stages — only a clinical exam and pocket-depth measurements tell us for certain, and that’s where every treatment plan starts.

What we do about it

How we treat periodontitis at Arasu Dental Care

We match the treatment to how far the disease has advanced — starting with the least invasive option that will actually control your case, and moving up only when the pockets and bone loss require it. Your care is led by our periodontist, , who holds an Associate Fellowship in Laser Dentistry from the World Clinical Laser Institute (WCLI), USA. Most treatment plans move through some of these steps:

1

The foundation of nearly every case

Deep cleaning (scaling & root planing)

We clean the plaque and hardened tartar from below the gumline — down inside the pockets a normal cleaning can’t reach — and smooth the root surfaces so the gum can reattach and the pockets can shrink. For early and many moderate cases, this alone brings the infection under control. It’s usually done over one or two visits under local anaesthesia, so it’s comfortable, and you’re back to normal eating within a day or two. Read more about our deep cleaning / scaling.

2

Minimally invasive, where it fits

Laser-assisted periodontal therapy

For suitable cases, we use dental lasers alongside deep cleaning to target infected tissue in the pockets and support healing, often with less bleeding and faster recovery than surgery. This is where our periodontist’s laser fellowship comes in — it lets us take a minimally invasive route for some patients who would otherwise need cutting. Whether a laser approach fits your case depends on the pocket depth and bone loss, and we’ll tell you honestly if it does or doesn’t. More on laser gum therapy.

3

For deep pockets

Flap surgery

When pockets are too deep to clean thoroughly from above, we perform flap surgery: the gum is gently lifted back so we can clean the roots and diseased tissue directly, then secured back into place to fit more snugly against the tooth. It sounds bigger than it feels — it’s done under local anaesthesia, and it’s the reliable way to save teeth that a deep clean alone can’t rescue. Learn more about our dental flap surgery.

4

For advanced cases

Regenerative procedures & gum grafting

In advanced cases, we can go a step further to rebuild what was lost — bone grafts, membranes that guide new tissue growth, or gum grafts to cover exposed roots and restore a stable gumline. These are for specific situations, not every patient, and we’ll only recommend them when the tooth is worth saving and the biology supports it.

5

The part that keeps it controlled

Periodontal maintenance

Periodontitis is a chronic condition, so treatment doesn’t end when the pockets close. To keep it from returning, most patients move onto a maintenance schedule — professional cleanings every three to four months instead of the usual six, plus a home routine we’ll coach you through. This is the single biggest factor in keeping your teeth long-term, and it’s why we build it into the plan from day one rather than leaving you to guess.

Treated by a periodontist, not referred out

Most gum treatment in the city is handled by general dentists. At Arasu, periodontitis is staged and treated by , whose Associate Fellowship in Laser Dentistry from the WCLI (USA) means we can offer minimally invasive, laser-assisted options alongside conventional surgery — not just refer them elsewhere. If your case also involves loose-tooth replacement, the implant and restorative team are under the same roof.

Why it happens

What causes periodontitis — and what raises your risk

At its root, periodontitis starts with plaque — the sticky bacterial film that builds up on teeth and hardens into tartar when it isn’t removed. Left along the gumline, it triggers the inflammation that slowly destroys the support around your teeth. But some things make it progress faster and hit harder, and knowing yours helps us plan your care:

What makes gum disease progress faster

  • Smoking and tobacco / paan use — one of the strongest risk factors; it also masks bleeding, so the disease advances quietly.
  • Diabetes — poorly controlled blood sugar and gum disease feed each other (more on this below).
  • Irregular cleaning or missed dental visits — the most common and most fixable cause.
  • Genetics, stress, and hormonal changes — which can make some people more prone even with good habits.

The cost of waiting

What happens if you leave it untreated

Periodontitis doesn’t stall — it advances. As more bone is lost, teeth loosen, shift, and eventually fall out or need to be removed, and periodontitis is one of the leading causes of tooth loss in adults. The infection can also flare into painful abscesses and spread to neighbouring teeth. This isn’t said to alarm you — it’s the honest reason we’d rather see you at the loose-tooth stage than the missing-tooth one.

If teeth have already been lost, we can talk about replacing them with dental implants once the gum disease is stabilised — but keeping your natural teeth, where we can, is almost always the better outcome. That’s the whole point of treating it now.

The honest part

Honest considerations before you start

Controlled, not cured

We’ll be straight with you: once periodontitis has caused bone loss, that stage can be halted and managed extremely well — but it isn’t reversed like a cavity is filled. The goal of treatment is to stop the damage, close the pockets, and keep the disease inactive for the long term. Gingivitis, caught earlier, genuinely can be reversed; periodontitis is managed.

A few realistic expectations worth setting

  • Some recession stays. Gums that have pulled back usually don’t return fully to where they were on their own, though grafting can restore some of it.
  • Maintenance is for life. Skipping the three-to-four-monthly cleanings is the most common reason gum disease comes back. Results depend heavily on this.
  • Not every tooth can be saved. Where support is too far gone, removal and replacement may be the more predictable choice — and we’ll tell you honestly which teeth are worth fighting for.

Individual results vary based on the stage of disease, your overall health, smoking status, and how consistently the maintenance routine is followed. A personalised assessment with our dentists is the only way to know what your case needs.

Beyond the mouth

Why treating your gums protects more than your smile

Gum disease isn’t sealed off inside your mouth. The clearest, most consistently researched connection is with diabetes, and it runs both ways: uncontrolled blood sugar makes gum disease worse, and active gum infection makes blood sugar harder to control. Periodontitis has also been linked in research to heart and other inflammatory conditions. For diabetic patients especially, getting the gums under control is part of managing overall health — which is one more reason we treat it seriously rather than as a cosmetic issue.

FAQs

Periodontitis: your questions answered

Not in the way a cavity is filled. Once bone has been lost, that stage is halted and managed rather than reversed — we stop the damage, close the pockets, and keep the disease inactive with maintenance. The earlier stage, gingivitis, genuinely can be reversed, which is why we’d rather catch it sooner.

The deep cleaning and surgical steps are done under local anaesthesia, so the procedure itself is comfortable. Any tenderness afterwards is usually mild and settles within a day or two, and for many patients the treatment relieves the soreness they arrived with.

Not necessarily. A tooth loosened by gum disease can often be saved if we treat the infection and, where needed, splint or surgically support it. Whether it’s savable depends on how much bone is left — we’ll tell you honestly at your assessment which teeth are worth keeping.

An early case controlled with deep cleaning may need just one or two visits plus reviews. Advanced cases involving surgery are staged over several appointments, followed by ongoing three-to-four-monthly maintenance cleanings.

Gingivitis is inflammation of the gums only, with no bone loss, and it’s reversible. Periodontitis is the next stage, where the infection has reached and started destroying the bone that supports your teeth — which is why it’s managed for the long term rather than simply cured.

Yes. Pyorrhea (பயரியா) is the common local name for advanced gum disease with pus and loosening teeth — the same condition dentists call periodontitis.

Yes, and it matters more than anything else. Regular maintenance cleanings, usually every three to four months, are the single biggest factor in keeping gum disease from returning. We build this into your plan from the start.

It can. The clearest link is with diabetes, and it runs both ways — active gum infection makes blood sugar harder to control, and vice versa. Getting the gums stable is part of managing overall health, especially for diabetic patients.
