Flap surgery is the next step only when pockets stay deep after deep cleaning has had a fair chance — because at that depth, instruments can no longer clean the root and bone reliably while working blind under the gum.

The Procedure
What happens during flap surgery at Arasu
Your treatment starts with periodontal charting and X-rays, so we operate on what is actually there rather than working blind. This plan is made by our periodontist, who holds an Associate Fellowship in Laser Dentistry from the World Clinical Laser Institute (USA) and performs our flap surgery and grafting cases. It is a real surgical procedure, but a routine, well-rehearsed one for her department.
Step 1
Numb & prepare
The area is fully numbed with local anaesthesia so you stay comfortable throughout — you feel pressure, not pain. Only the quadrant being treated is worked on at a time.
Step 2
Lift the flap
The gum is gently lifted away from the tooth as a flap, exposing the root surfaces and the true depth of the pockets — the access a blind cleaning simply can't give.
Step 3
Clean & reshape
Infection and hardened tartar are removed under direct vision; damaged bone is smoothed or grafted where indicated, and laser-assisted decontamination is added where it helps healing.
Step 4
Suture & heal
The gum is secured snugly back against the tooth with fine sutures so pockets are shallower and easier to keep clean, and you're sent home with a simple recovery plan.

Any swelling peaks and then settles. Most patients are back to work and their normal routine by now, keeping the area clean as advised.

What it does
Flap surgery reaches the deep infection a cleaning can't, reduces pocket depth so you can keep the area clean at home, and stops the ongoing bone loss that loosens and eventually loses teeth. For the right case it is often the difference between keeping a tooth and needing an extraction — and where bone has been lost, a graft during surgery can help rebuild support.
What it can't do
It isn't a cure for gum disease — periodontitis is managed for life with maintenance, or the bacteria repopulate. Gum and bone already lost don't fully regrow on their own. If a deep cleaning is still working for you, surgery isn't needed yet — start with scaling & root planing. And where recession has exposed roots, gum grafting is the treatment that covers them — flap surgery won't.
