When it's the right call
Does your child need a pulpectomy?
A pulpectomy is the right treatment when infection has gone deep — into the nerve and root canals — but there's still enough healthy tooth to rebuild and keep. We confirm this with a quick examination and a small X-ray before we start. Deep decay in young children is more common than most parents expect: across Indian studies, close to half of children have some decay in their baby teeth — and caught in time, a deep cavity like this can usually be treated in a single appointment rather than ending in an extraction.
Your child may need a baby tooth root canal if you're noticing:
A toothache that keeps coming back — especially pain at night, or pain bad enough to wake them or stop them eating.
A tooth that's turned grey or dark — often a sign the nerve inside has been damaged.
Swelling on the gum or face, or a small pus-filled bump near the tooth — this points to an abscess and needs to be seen quickly.
A deep cavity that reaches the nerve, or a cracked or knocked tooth from a fall that has exposed the pulp.
Lingering sensitivity to hot or cold that doesn't settle after a few seconds.
How we decide
Pulpotomy or pulpectomy — which one does my child need?
Baby-tooth pulp treatment comes in two forms, and the difference is simply how deep the infection has reached. It's the first thing we work out from the X-ray.

If only the top of the pulp is affected, a pulpotomy is enough and preserves more of the living tooth. Once the infection reaches the roots, a pulpectomy is needed to clear it fully. Both are done in a single appointment; your child's dentist decides between them based on what the X-ray shows on the day.
What to expect
What's involved: the treatment, step by step
We plan every baby tooth root canal to be calm, quick, and — as far as possible — a positive visit for your child. Start to finish, it usually takes 45 minutes to an hour, in a single appointment. Here's exactly what we do:
STEP 1
We check and take an X-ray
This confirms how far the infection has spread and that a pulpectomy is the right call for the tooth.
STEP 2
We numb the tooth gently
The area is fully numbed with local anaesthesia so your child is comfortable throughout. For an anxious or very young child, we can add sedation to help them relax.
STEP 3
We keep the tooth clean and dry
A soft rubber sheet — a dental dam — isolates the tooth so treatment stays clean throughout.
STEP 4
We remove the infection
We take out the infected pulp from the crown and the root canals, then clean and disinfect the canals to clear the bacteria causing the pain.
STEP 5
We fill the canals
We fill the canals with a special paste that the body absorbs naturally as the adult tooth develops — so the baby tooth can still fall out on its own at the right time.
STEP 6
We cap the tooth with a crown
A treated tooth is more fragile, so we almost always protect it with a small crown for children so it can chew and last normally.
Modern techniques have made this far more predictable than the old "root canal" reputation suggests. In recent studies using rotary instruments — the kind of approach we use — baby tooth root canals succeeded in around 96% of cases at six-month follow-up. Recovery is usually quick: some mild soreness for a day or two, managed with children's pain relief if needed, and soft foods to start. Most children are back to their normal routine by the next day.

- Success is high, but not certain for every tooth. How well a baby tooth root canal holds depends mostly on how much healthy root is left and how completely the canals are cleaned and sealed. Across long-term studies, success has ranged widely — from under 40% for badly damaged, heavily resorbing teeth to over 95% for teeth treated in good time. That's exactly why we assess each tooth carefully first rather than promising an outcome.
- The crown is part of the treatment, not an extra. A treated baby tooth left without a crown is far more likely to fracture or reinfect, so we include capping it in the plan.
- Some teeth are past saving. If too little healthy structure remains, the honest answer is to remove the tooth and hold the space with a space maintainer so the adult tooth still comes through in the right place.
- Results vary from child to child. Outcomes depend on the tooth, the extent of infection, and home care afterwards.
Keeping the natural tooth is almost always the better outcome when it's possible: it protects the space for the adult tooth, keeps chewing and speech on track, and avoids the extra step of a space maintainer later.
