Is it really recession — and does it need treatment?

Recession is easy to miss early because it usually doesn’t hurt. The signs patients actually notice are worth knowing, because they tell us how far it has gone:

Signs your gums may be receding

  • Teeth that look longer, or a gumline that looks uneven from one tooth to the next.
  • A small step or notch you can feel with your tongue or nail where the tooth meets the gum.
  • Sudden sensitivity to cold, sweet or air — the exposed root has no enamel to protect it.
  • A tooth that looks slightly darker or yellower near the gumline — that’s root surface showing.
  • Gums that bleed, look puffy, or have pulled away, which points to gum disease driving the recession.

Not every case needs surgery. Mild recession that isn’t progressing can often be managed by fixing what’s causing it. But recession that keeps advancing, exposes root, causes ongoing sensitivity, or sits alongside active gum disease does need treatment — left alone, it can reach the point where the tooth loosens. Knowing the cause matters, because it decides the treatment. In our experience the recession we see falls into a few clear groups:

What drives gum recession

  • Brushing too hard, or with a hard brush. This is the single most common cause we see, and it’s backed by the research — across thirteen studies, aggressive and horizontal scrubbing was the toothbrushing factor most often linked to recession. It wears the gum back mechanically, usually on the cheek side of the teeth you brush hardest.
  • Gum disease. Gingivitis and periodontitis destroy the gum and bone that hold the tooth, and the gum recedes as that support is lost. Here recession is a symptom of active infection that has to be treated first.
  • Grinding, clenching and bite stress. Constant pressure on a tooth can push the gum back over time.
  • Position and anatomy. Teeth that sit forward in the arch, thin gum tissue, or a high muscle attachment (frenum) pulling on the gum all make recession more likely.
  • Other triggers — smoking and tobacco, tongue or lip piercings, and past orthodontic movement can all contribute.

Gum grafting is the treatment most people picture when they think about “fixing” receding gums, and the results are strong when it’s indicated: long-term studies of connective-tissue grafts report around 93% root coverage at one year and 95–96% holding at five to ten years. What’s honest to expect

Gums that have receded do not grow back on their own. This is the honest starting point. No mouthwash, gel or brushing change regrows lost gum tissue — those things can stop it getting worse, but only a graft can physically cover an exposed root again. Anyone promising that receding gums will “regrow” without treatment isn’t being straight with you.

Earlier is easier. The further recession advances — especially once the bone between teeth is lost — the harder full root coverage becomes and the less predictable the result. Treating it while it’s still limited to the gum gives a more complete and more stable outcome, which is exactly why we don’t recommend “wait and see” once it’s clearly progressing.

Results vary, and we’ll tell you your case honestly. Root-coverage success depends on the type and stage of recession, your gum thickness, and how well the cause is controlled afterwards. For some people we can expect near-complete coverage; for others the realistic goal is to halt the recession, reduce sensitivity and thicken the gum rather than fully cover the root.

No — gum tissue that has already receded does not grow back on its own, and no toothpaste, gel or mouthwash regrows it. What you can do is stop it progressing by fixing the cause, and where the root is exposed, a gum graft can physically cover it again. That’s why early treatment matters.

Gum grafting is performed under local anaesthesia so you won’t feel it being done; afterwards there’s manageable soreness for a few days that settles with prescribed medication.
